Before Admission

Registration

We want your day surgery admission to run smoothly, so please register your details with us well before your planned admission date.

The quickest and easiest way to do this is via our secure online admission, which can be completed on a desktop computer, tablet or smartphone.

Please have the following information ready when you fill in your admission form:

1. Person to contact details (e.g. family member, friend, carer)
2. Medicare number
3. Funding details (e.g. DVA, Private Health Fund, insurance claim or self funding)
4. Benefit details (e.g. Pension number)
5. Medications
6. Your height and weight

If you are not familiar with computers, a family member or friend may be able to assist you.  You can also make an appointment to come in and one of our friendly admission staff will help you to complete the online admission.

Online registration will take approximately 30 minutes, and if you don’t have time to complete the form in one sitting you can save it at any time to finish later.

Alternatively, you can telephone the day surgery on 02 9553 9905 during business hours and we will mail the forms to you, or you can download the forms via the link below.  Once completed, please submit to the day surgery as soon as possible prior to your admission date.

Pre-Admission

Please telephone (02) 9553 9905 the working day before your admission during office hours.

Allow 10 minutes for this call to confirm the details of your admission.  You will need to telephone each time you are admitted.

If you will be travelling by Community Transport to and from the day surgery, please telephone two working days prior to the date of admission.
